guess that poem part 2

quick on the heels of the last one, and in honor of not seeing my Dad in a long time. This time you have to name the play it's from as well as the author.

Full fathom five thy father lies;
Of his bones are coral made;
Those are pearls that were his eyes;
Nothing of him that doth fade
But doth suffer a sea-change
Into something rich and strange.
